About this course:
This course explores how clinical documentation directly impacts dental insurance claims, patient care continuity, and practice compliance. Participants will learn why incomplete or vague chart notes are one of the most common causes of claim denials and delayed reimbursement, and how stronger documentation practices can improve financial outcomes and reduce administrative burden. Using real-world examples and the SOAP documentation framework (Subjective, Objective, Assessment, Plan), the course demonstrates how dental teams can create clear, defensible clinical notes that support both patient care and insurance reimbursement. Attendees will also learn how different roles within the practice contribute to effective documentation and how standardized templates and workflows can improve efficiency and accuracy.
Learning Objectives:
Upon completion of this course, participants will be able to:
- Explain the relationship between clinical documentation and claim approvals,
including common reasons insurance claims are denied due to inadequate documentation. - Describe the SOAP documentation framework (Subjective, Objective, Assessment,

administrative staff to improve team communication and record accuracy. - Construct improved clinical notes using the SOAP format to demonstrate medical
